Autorización de datos / Data authorization

Los datos solicitados serán incorporados a un sistema de tratamiento de datos de carácter personal titularidad de Global Football Total S.L. y protegidos conforme a lo dispuesto en la Ley 15/1999, de Protección de Datos de Carácter Personal y normativa de desarrollo.

El/la alumno/a y/o su representante legal autorizan a Global Football Total SL al uso y publicación de fotografías y todo tipo de imágenes del mismo en la página web y redes sociales de Global Football Total S.L. y del LEVANTE UD.

Caso de tratarse de datos personales de menores de 14 años, el consentimiento para este tratamiento de datos personales y cesión de los mismos se entenderá prestado por sus padres, tutores o representantes legales, que serán quienes necesariamente deban suscribir y firmar el formulario de recogida de datos al que se adjunta la presente cláusula informativa.

El titular de los datos, o en su caso sus padres o tutores legales, podrán ejercer los derechos de acceso, cancelación, rectificación y oposición, enviando un escrito al efecto dirigido a Global Football Total S L , Avenida Cortes Valencianas 26 (46015 Valencia) o al correo electrónico a manager@globalfootballtotal.com.

Los datos personales podrán ser empleados para el envío por Global Football Total de comunicaciones por medios postales o electrónicos.

Soy conocedor, por así haberme informado GLOBAL FOOTBALL TOTAL, además de ser de conocimiento general, que mi hijo/a mientras reciba dicha formación, en principio no podrá disputar partidos oficiales, dado que la actual normativa FIFA prohíbe la inscripción federativa de jugadores extranjeros en la competición nacional española, por lo que reconozco que la formación deportiva se limitará a entrenamientos y partidos amistosos. / I am knowledgeable, for having informed me GLOBAL FOOTBALL TOTAL, in addition to being of general knowledge, that while my son / daughter receives this training, in principle he / she will not be able to play official matches, given that the current FIFA regulations prohibit the federative registration of foreign players in the Spanish national competition, so I recognize that sports training will be limited to training sessions and friendly matches.

Por todo lo anterior, y a los efectos oportunos, eximo al LEVANTE U.D., S.A.D., ante cualquier reclamación que pudiera recibir de otro club o sociedad deportiva de cualquier país por el eventual conflicto que pudiera suscitarse por los derechos federativos de mi hijo/a. / For all of the above, and for the appropriate purposes, I exempt LEVANTE U.D., S.A.D., from any claim that it may receive from another club or sports society in any country for the eventual conflict that may arise due to the federative rights of my son / daughter.

Si ha pagado el importe del programa, pero no ha podido completar todo el período de formación y no ha podido disfrutar de los servicios contratados como alojamiento,comidas y transporte, que corresponden al pago realizado, GLOBAL FOOTBALL TOTAL, no devolverá el pago realizado en ningún caso. Si, guardará, durante un año desde la salida del jugador del programa, el período de tiempo formativo y de servicios no satisfecho por el jugador, para que cuando pueda y ambas partes lo acuerden, éste pueda retomar el programa. / 1. Si ha pagado el importe del programa, pero no ha podido completar todo el período de formación y no ha podido disfrutar de los servicios contratados como alojamiento,comidas y transporte, que corresponden al pago realizado, y a su vez no quiere volver a retomar el programa en ningún momento, el jugador junto con su familia deberá enviar un email a manager@globalfootballtotal , explicando los motivos de esa decisión. Una vez recibido el email, el cómite ejecutivo del programa GLOBAL FOOTBALL TOTAL, estudiará la solicitud y en el caso de aceptarla, realizará una devolución sobre el período pagado y no disfrutado, que variará su importe dependiendo del tiempo que quede para completar su programa, en el momento de la salida del mismo: / 2. *Todos estos supuestos tendrán validez, siempre y cuando el programa GLOBAL FOOTBALL TOTAL junto con el LEVANTE U.D. Hemos sido informados con anterioridad a la suscripción del presente documento, por parte de GLOBAL FOOTBALL TOTAL S.L., del importe del programa contratado y de las condiciones de pago, que aceptamos de manera expresa, comprometiéndonos a cumplirlas en los plazos convenidos por las partes. / We have been informed prior to the signing of this document, by GLOBAL FOOTBALL TOTAL S.L., of the amount of the contracted program and of the payment conditions, which we expressly accept, committing ourselves to comply with them within the terms agreed by the parties.

Manifestamos haber contratado un seguro medico con asistencia en España del que es beneficiario nuestro hijo/a. Su estado de salud es bueno, y garantizamos que está preparado para la actividad deportiva. / We state that we have contracted medical insurance with assistance in Spain of which our son / daughter is the beneficiary. Nos comprometemos a no perjudicar a GLOBAL FOOTBALL TOTAL S.L. y al LEVANTE U.D. SAD en cualquiera de las acciones formativo-deportivas, sabiendo que los jugadores pueden ser expulsados de la participación en los programas, con la posible pérdida de pago o cuotas, por conducta violenta o comportamiento antideportivo dentro o fuera del campo de jugar. / We promise not to harm GLOBAL FOOTBALL TOTAL S.L. and LEVANTE U.D. SAD in any of the training-sports actions, knowing that the players can be expelled from participation in the programs, with the possible loss of payment or fees, for violent conduct or unsportsmanlike behavior on or off the playing field.

Aceptamos pagar todos y cada uno de los daños y perjuicios a cualquier propiedad o indemnización causada voluntariamente por mi hijo/a, de una manera negligente o de otro tipo. El fútbol es un deporte físico, de contacto, que implica el riesgo de lesiones, por ello asumimos todos los riesgos y peligros asociados con la participación de mi hijo/a en la actividad que va a realizar. / We agree to pay any and all damages to any property or compensation voluntarily caused by my child, in a negligent or other manner. Informo, que el estado de salud de mi hijo/a es bueno, y por tanto, garantizo que está preparado/a para la actividad deportiva a realizar en el programa GLOBAL FOOTBALL TOTAL. Por todo ello, me comprometo a que mi hijo o hija no perjudicará a GLOBAL FOOTBALL TOTAL S.L. y al LEVANTE U.D. SAD en cualquiera de las acciones formativo-deportivas a realizar, y cumplirá el código disciplinario y ético del régimen interno del programa y de la residencia de jugadores, sabiendo que el incumplimiento de este código, puede conllevarle una sanción e incluso la expulsión del programa, si así se considerara en función de la resolución del cómite ejecutivo del programa. Soy conocedor que la expulsión del programa supondrá la pérdida 100 % de los pagos o cuotas que haya realizado y haya contratado y no haya podido disfrutar debido a la sanción. / I inform that my son / daughter's health status is good, and therefore, I guarantee that he / she is prepared for the sports activity to be carried out in the GLOBAL FOOTBALL TOTAL program. Al mismo tiempo si mi hijo/a es sancionado/a con una sanción deportiva, el programa GLOBAL FOOTBALL TOTAL, no me reembolsará dinero alguno por la no realización de las acciones deportivas, durante el período de sanción. / At the same time if my child is sanctioned with a sports sanction, The GLOBAL FOOTBALL TOTAL program will not reimburse me for any non-performance of sports actions, during the penalty period.
